Physical Therapist- Early Intervention, 37.5 Hrs- Days This position provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay. Services are designed to meet the needs of each infant or toddler, and the needs of their family. Clinician, Youth Mobile Crisis - 24 Hours Nights The Access Division provides clinical support 7 days per week, 24 hours per day to individuals experiencing a mental health and/or substance abuse problem. Supports clients in need in the community (CHL sites, area hospitals and/or other community locations) or via telehealth. Performs assessments, safety/crisis planning, and referrals for further treatment/services as assigned. Clinicians are responsible for engaging with individuals to complete assessments, develop behavioral health treatment plans, participate in brief interventions, and aftercare planning. Clinician-Family Support & Stabilization, 37.5-Days 1. Conducts eligibility screenings and comprehensive intake and follow up assessments to assess eligibility, gather clinical information, and inform treatment planning.Clinician-Family Support & Stabilization, 37.5-Days
2. Conducts client evaluations including bio-psychosocial assessment, diagnostic formulation, and initial treatment plan recommendations.
3. Meets with clients to provide individual counseling and assist clients in attaining treatment goals.
4. Writes treatment plans including client’s strengths with behaviorally based goals and objectives. Documents service delivery in accordance with department standards and progress of goals and objectives in progress notes. Refers clients to resources with and outside CHL. -
Community Crisis Stabilization Clinician - Part Time/Weekend Provides counseling services to clients with substance abuse and mental health issues. Conducts diagnostic evaluations, writes behavioral treatment plans, provides individual counseling, writes aftercare plans in accordance with department quality and productivity standards.
